How to use the Universal Healthcare State Program at Evex:

Visit any Evex clinic, present a valid ID, and complete registration. Please note that in the case of a minor, it is mandatory to present the child’s birth certificate along with the parent’s/guardian’s ID or passport.

Information on alternative funding sources

For services not covered by the state program, you may obtain additional funding. Contact an Evex family doctor and, based on the completed Form 100, request funding for the required medical examinations from the Ministry of Internally Displaced Persons from the Occupied Territories, Labour, Health and Social Affairs of Georgia, local district administrations, or the city municipality. This option applies to both planned and emergency surgeries, as well as other high-technology examinations.
